An Active Search for Pilot Partners: Understanding the Use of Social Determinants of Health Screening Tools

To gain a more comprehensive understanding of the collection landscape and ingestion of Social Determinants of Health (SDOH) data, HealtheConnections is working with all the QEs and the Gravity Project, a national public collaborative that develops consensus-based data standards to improve how we use and share information on social determinants of health (SDOH).

We are actively seeking participants to help us pilot the exchange of data from Social Determinants of Health screening tools. As a first step, we are surveying partners about their current use of these screening tools. If you received an email from us to fill out a survey, please fill it out as soon as possible.

Collaboration with interested partners around the exchange of social needs data is essential as the 1115 Waiver continues to be a focus for HealtheConnections, other QEs across the state, and our state partners.
