HealtheConnections, NYS-HCCN and Bronx RHIO Host Learning Collaborative

HealtheConnections is partnering with NYS-HCCN and the Bronx RHIO to host a small group learning collaborative on the topic of HIE considerations for Race, Ethnicity and Language (REaL) Data Collection.
Collection of REaL data is critical for stratifying populations and measures to identify health disparities and gaps in clinical care. By participating in this practice facilitation, CHCs can increase the volume and improve the completeness of this data for their patient population and in the statewide health information exchange (HIE).
The objectives of this learning collaborative are to:
  • Build health center awareness of the flow of REaL data from EHR collection to the HIE
  • Grow EHR capacity to collect REaL data
  • Explore decision-making scenarios for data collected or received
  • Identify and discuss technology-related challenges and best practices for collection
