Mid-Hudson Region Community Health Assessment 2019-2021: A collaborative made up of seven county health departments (Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster, and Westchester) and 17 hospitals in the Mid-Hudson region worked together to compile a singular, regional community health assessment (CHA) for 2019-2021. The purpose of the CHA is to present data about local health needs within the region, and to subsequently utilize this information to create interventions that will improve the overall health of community residents.
Road Map to Health Equity: The Road Map to Health Equity was developed as a tool to improve institutional and professional development in the areas of the social determinants of health and health equity to help organizations better address health disparities in their communities. It is meant to connect those interested in addressing racism and inequity to resources that will help on their journey while bringing others on board.
