New York State Disability and Health Initiative

HealtheConnections serves as a subcontractor to the New York State Department of Health on a CDC initiative designed to improve the health and quality of life among people with mobility limitations and/or intellectual disabilities through adaptation and implementation of evidence-based strategies. In this role, HealtheConnections works with local community-based organizations by promoting inclusive policy, systems and environmental change strategies to improve access to physical activity and healthy foods for people with disabilities.

New York State Diabetes, Heart Disease and Stroke Innovation Initiative

HealtheConnections serves as a subcontractor to the New York State Department of Health on a CDC initiative designed to test and evaluate innovative approaches to address diabetes, heart disease and stroke. In this role, HealtheConnections supports a Diabetes Prevention Program (DPP) assistance center, building capacity and providing technical assistance and support to organizations offering the DPP. HealtheConnections is also implementing systems to facilitate bi-directional e-referral between healthcare systems and community initiatives, and building tools to use HIE data to identify patients with undiagnosed hypertension and pre-diabetes.

New York State Arthritis Initiative

HealtheConnections serves as a subcontractor to the New York State Department of Health on a CDC initiative designed to improve the health status of adults with arthritis by promoting evidence-based interventions and community walking initiatives. In this role, HealtheConnections will implement a counseling and referral program with primary care providers to promote the importance of physical activity for people with arthritis and refer patients to community programs.
