SHIN-NY Customer Usage and Satisfaction Survey sent by the NORC University of Chicago will send a survey to a statistically sampled group of participants about our health information exchange services, account management, and overall experience as a HealtheConnections participant.

This survey will come directly from the SHIN-NY Survey Team at NORC, on behalf of the New York eHealth Collaborative (NYeC). Only a sample of our network will receive this survey, so you may not receive one.
