Updated Consent Form Available for RHIO Administrators
Includes important language regarding substance abuse treatment
In preparation for the integration of 42 CFR Part 2 (Substance Use Disorder) data, HealtheConnections has been rolling out an updated consent form to the RHIO Administrators*. The form includes required language to make the patient aware that giving a “Yes” consent will allow the authorized users at a participating organization to access and view Part 2 data from drug and alcohol treatment centers that are contributing to the health information exchange.
The new language below has been added to the second page of the consent form, under the second item:
If you have received alcohol or drug abuse care, your record may include information related to your alcohol or drug abuse diagnoses, medications and dosages, lab tests, allergies, substance use history, trauma history, hospital discharges, employment, living situation and social supports, and health insurance claims history.
While the consent form has been sent out in English, additional language translations will be available in the next week. If you need the consent form in another language, RHIO Administrators can contact their Community Engagement Specialist or send an email to support@healtheconnections.org.
*Reminder for RHIO Administrators to return their attestation form, indicating that you received the updated consent form and have distributed to the appropriate users within your organization.
