New Type of Data Now Being Contributed to the HIE
HealtheConnections’ participants that contribute data through Medent will now be supplying the UD (Universal Document). Also known as “Progress Notes”, this important information contains a summary of the patient’s encounter/visit with the provider. These documents are separate from, and will supplement the Continuity of Care Document (CCD) that HeC already receives. The Progress Notes are similar to the dictated transcription and radiology reports contributed to HeC from hospitals, labs, and imaging centers. You will be able to locate this information on the “Documents” tab in the Patient Lookup service in HealtheConnections.
County Health Departments Begin Contributing Data
Jefferson County Public Health Service and Cortland County Health Department have joined the growing list of data contributors to the health information exchange. Each of these organizations is now contributing Continuity of Care (CCD) documents that contain patient demographics, diagnoses, procedures, vitals, allergies, medications, and much more from patient encounters through healthcare services offered by the local health departments. HealtheConnections would like to recognize the efforts made by these agencies as the first public health entities to integrate their electronic health records and share data with the health information exchange.
