Sarah Hudson is a 24-year old with schizophrenia and high blood pressure.  She receives Medicaid Health Home care management services to help her manage both her mental health and physical health conditions.  One day, Sarah had a regularly scheduled appointment with her care manager and she didn’t show up for the appointment.  Concerned about her absence and general well-being, the care manager searched Sarah’s medical records using Patient Lookup and saw an Emergency Department registration alert from the night before.  Sarah had gotten confused and wandered into the street and was hit by a car.  The care manager also saw a hospital inpatient admission alert and was able to see that Sarah was admitted into the local hospital nearby.  Because Sarah had been brought in by ambulance and had been unconscious for some time, the hospital did not have detailed information about her mental and physical health conditions.  The care manager was able to visit Sarah in the hospital and work with hospital staff to review pharmacy and medication lists and to plan the appropriate follow up care for her upon discharge from the hospital.  The care manager was able to send a secure email with information about the incident and hospital visit to Sarah’s psychiatrist at the local FQHC for awareness.

Once the care manager had received notification about Sarah through myAlerts, she was able to access Patient Lookup and see details about Sarah’s Emergency Department visit the night before.  She would not have known what happened to Sarah or how to reach her if she didn’t have access to the HIE.  Being able to track high-risk patients is a critical component in managing and improving care for these individuals.  The care manager was also able to educate the hospital staff on Sarah’s diagnoses and medications which reduced the number of days that Sarah needed to stay in the hospital.  The care manager used Direct Mail to notify Sarah’s doctor of her incident and medications prescribed at the hospital.
