HealtheConnections Health Planning is helping to make Central New York the healthiest region in New York State.
The value of regional health planning and regional collaborations like HealtheConnections has been affirmed by the New York State Medicaid Redesign Team, the Public Health and Health Planning Council CON Redesign Plan, and Centers for Medicare and Medicaid State Innovation Models.
Last year, some of our staff served on the ‘indicators’ work group with the New York State Department of Health (NYSDOH) prior to the release of the 2013-17 Prevention Agenda: New York State’s Health Improvement Plan. The Health Improvement Plan is a comprehensive five-year plan to improve the health and quality of life for everyone who lives in New York. The plan is a blueprint for local community action to improve health and address health disparities. In 2013, HealtheConnections Health Planning was funded for Supporting Local Community Health Improvement. Working with county public health departments, rural health networks, hospitals and community groups we helped to facilitate completion of a three-part planning process throughout the Central New York region. There are many life style, societal and economic factors that influence disparities in health outcomes, but across the region, access to healthcare is an important variable. HealtheConnections Health Planning is addressing this issue in two ways.
Click the Practitioner Database button for information on a project funded by the New York State Health Foundation to develop a searchable integrated database of medical practitioners. Central New York needs to increase our pool of primary care practitioners to be able to serve the increasing number of new people who will access healthcare due to new insurance coverage options, and our aging neighbors who will need to access healthcare more frequently.

HealtheConnections Health Planning is the lead agency for Navigator services in Onondaga County working with our partners: Benefit Specialists of New York, Northside Urban Partnership, South Side Innovation Center and The Salvation Army.
