Home > patients > Providing Consent Providing ConsentIn order to participate in HealtheConnections, you must provide consent to EACH of your health care providers. Consent is entity-based, meaning that when you sign a HealtheConnections™ Consent form for an organization, you are allowing all their providers and staff members that are involved in your care and authorized users access to your information on HealtheConnections™. By granting consent, medical providers treating you will have access to important clinical information about you that could be unavailable otherwise. For more specific information, visit the Consent Frequently Asked Questions. There are two ways you can establish and communicate your intentions regarding consent:
If you have already consented to participate in HealtheConnections™ and you wish to withdraw consent, you can do so by completing one of the two following steps:
1. Return to the participating provider's office where you consented and ask to complete the HealtheConnections™ withdrawal consent form. They will process the form for you.
2. Complete the form outside of the provider’s office and either bring it back or mail it to their office. |
